FORM FOR REQUEST FOR BIRTH CERTIFICATE

 

Steps to obtain a Birth Certificate in the Democratic Republic of Congo:

 

1. Obtain a supplementary judgment: (If you do not have a birth certificate)

• Go to the competent court.

• Submit a request for a supplementary judgment with supporting documents (identity, testimonies, etc.).

• Wait for the court's decision.

 

2. Request for a full copy of the birth certificate and the birth certificate:

• Go to the civil registry office.

• Complete the full copy request form.

• Provide identity documents and, if available, information about birth (date, place, parents' names).

• Submit the birth certificate or the supplementary judgment to the civil status office to have the birth certificate drawn up

 

3. Legalization by a notary:

- Go to a notary with the documents (supplementary judgment and birth certificate).

- Request legalization of documents to guarantee their authenticity.
